Thank you for choosing Associates of Internal Medicine for your medical care! We look forward to meeting you and hope that Associates of Internal Medicine will meet all of your health care needs.

We have made the New Patient Forms available to you online in a PDF. Please complete these forms and bring them with you to your initial appointment. The completion of the Health Care Advanced Directives (Living Will, Power of Attorney, Health Care Proxy, etc.) is optional. However, we highly recommend providing our office with a copy so that we can keep it in your file.

A FEW ITEMS TO REMEMBER FOR YOUR FIRST APPOINTMENT:

• After completing our medical records request form please either fax it to 561-819-6466 or drop it off at our office so that we can obtain your medical records.
• Along with your new patient paperwork and advanced directive, please bring a list of your current medications, a list of all the specialist you see, and your insurance card and photo ID.
• Arrive 15 minutes prior to your appointment.
• If you are unable to keep your appointment, please contact our office 24 hours in advance to reschedule at 561-498-7474.
